申请访加目的说明书

我妈五年内来过加拿大，今年又想申请她来度夏，但不知“申请访加目的说明书”怎么写。有谁知道是用英文写给签证官吗？有模板吗？

所有时间均为格林尼治时间 -4。现在的时间是 22:22。
