THAT I am the spouse of the Defendant, whose last known address is XXXXXX in the City of Calgary, in the Province of Alberta .
2. MARRIAGE
THAT I was married to the Defendant, XXX, on the xx day of xx, 2003, at the City of Guiyang, in the Province of Guihou, P.R.China. We got married at the Bureau of Civil Affairs in Guangzhou. We both gave the oath. The marriage was certified by the Civil Affair Officer.
请问：
1，Affidavit of Applicant part #1 #2 应怎样改写。尤其是“We got married at the Bureau of Civil Affairs in Guangzhou. We both gave the oath. The marriage was certified by the Civil Affair Officer.”不被认为是solemn form，怎么办？
2，Divorce Judgment 的签字栏页上，必须同时包括哪些内容，应如何排版，以避免“某某页信息空白···”云云
3，我是否可以重新递交Affidavit of Applicant和Divorce Judgment请求重新审理，还是必须全部重来？
