【求助】One Q. about parents visiting.

Ask: when inviting parents to visit, there are two forms that need to be filled, DETAILS OF EDUCATION and FAMILY COMPOSITION. Do they have to be filled in both English and Chinese or Chinese only if fine? Thanks a lot!
