没说一定需要，而且你想想原件都丢了还怎么办“certified true copy”？还有那个东西是不补发的。

需要补办，但不影响入籍。
我和我夫人2006年3月份入籍宣誓。宣誓当日均需上交落地纸，我们坦白说弄丢了。
Officer很nice, 他要求我们每人填写一张“丢纸说明”申明原因。我们写的是“以为拿到了枫叶卡，落地纸就没用了，所以销毁了”。
落地纸一定要补办，因为会影响到退休后的pension(养老金)。
Officer当时告诉我们如何补办，可惜现在记不太清楚了。好像是从网站上下载表格，每人补交$30。

最好是到附近的警察局报个案,这个是很重要的文件,我当时去的时候,警察作了很详细的记录,然后给我一个REPORT NUMBER,这个号码最好是写在你申请Certified True Copy上的.去入籍的时候,带着警察局给你复印的REPORT,给移民官看就行了,不会有什么麻烦.
补办的手续也很简单,上面有人说了,$30,添表寄去,12周会收到.

请问哪位知道是哪一个表格？谢谢

准备申请入籍，看了一下申请材料，需要移民纸和枫叶卡的copy件，可老公的移民纸弄丢了，怎么办？先补办再申请？急！谢谢！

找个办证的，什么证都好办，这里找不到，到中国找。
