和老公商量的结果，准备给七岁的孩子改LAST NAME，孩子是担保移民过来的，熟知加国法律的大虾们，指点一下，要有哪些步骤？

Change of Name

Formal name change (Children)
A formal change of name involves legally changing a person´s first and/or second name and/or the individual´s last name.

To formally change a child´s name, s/he must be under the age of 18 and have lived in Ontario for at least one year before submitting a change of name application.

Please note that there are different procedures for changing a child´s name depending on the age of the child, the name you choose and who has custody of the child.

When you change a child´s name, you will be issued a birth certificate indicating the new legal name(s).

Note: A formal change of name is processed in the order in which it is received. See Service Times.

A note about custody & access: If you have sole custody, others may still be entitled to access of the child. If a court order is silent on the issue of access, you will be required to provide notice to the other persons that have access to the child.

the surname of a child may be changed only to a surname that:
(1) is the surname or maiden surname of the mother;
(2) is the surname of the father;
(3) is the surname of the spouse or AIP of the mother or of the father.
