yes,you have to tell the agent,and let the company know this as well.
otherwise, the company knows it later anyway and it's not good if that happens.

if the company (not the agent) has known this and still wants you,that's your second chance.
if the agent knows it but doesn't let the company know,you are the victim or you become statistics; before you go, make sure the agent has told tell them.

you failed a paper test, not face to face interview,i don't think they could remember you based on your test.

a face-to-face is of coures defferent.

I don't see the point of not letting them know; what if they don't ask you why ,but just wonder in their mind about your honesty?
you lose nothing when you tell them and if they don't want you,you save time too.it's much better than later a surprise.
