想给学医的朋友办一次免费讲座，先统计一下这里的医生人数，请大家回贴。

加拿大有100万华人，大家一定觉得奇怪，为什么很少看到中国大陆医学院毕业的医生在加拿大行医。（请注意，中医在这里称作草药和针灸治疗，不需要行医执照，本地白人只需要学习3个月，拿到简单的开业执照就可以开一个针灸诊所，而且大多数中药属于禁用药物，例如朱砂、大黄、一些含铅、汞等的中药都被西医认为是毒药，这里的中医开的药方和国内的相差甚远，可供选择的草药不及十分之一，所以下面我们讨论的内容不包括中医）。

回到上面的话题，加拿大的华人医生要么是移民或者第二代移民在本地读的医学院，要么就是香港医生，为什么看不到中国大陆医学院毕业的医生呢？

我认为有下面几个原因，首先，在加拿大行医是很难的，难到我们望而生畏。网上刚刚有人问一句，“请问国内10年经验的主治医生在加拿大有什么前途”？下面就会出现十几个甚至更多劝告的回贴，“忘了你是医生吧”，“根本没有可能”，“改行做护士”，“找一家医学实验室当技术员已经算很不错的了”，等等。说这些话的人大多数并不是医生，不了解如何拿到行医执照，但他们是好心，因为他们没有接触过已经成功的医生，甚至连听说都没有听说过。相信大家都知道王又辛前辈的家书，曾经给了我们自办移民很多指导，他夫人是北医大毕业的，96年过来以后发现行医太难，改行学电脑，99年当了程序员。我当时也是这样认为的，所以迟迟下不了决心，直到LD给了我大力支持，才抱着试试看的心理走上了这条路，谁知一发不可回头。虽然我知道学医没有尽头，因此直到现在也没有成功的喜悦，但是我还是庆幸选择了自己热爱的事业，如果没有LD的付出，如果我当初知难而退......保尔.柯查金是怎么说的来着？“人的一生应该这样度过......”

其次，学医的朋友们缺少相关信息，没有前人的经验可以参考，造成自己缺乏信心。我接触到的好几个医生都说，“还没有听说中国人谁考过行医执照呢，本地白人想进去都那么困难，何况咱们英语远不如他们”。我知道最近每年都有至少一位大陆毕业的医生在安省当上住院医生，我自己是今年刚刚拿到住院医生位子，大多数人都是靠自己打拼，最开始的时候也没有信心，缺乏信息，走了很多弯路。学医很苦，没有时间刷网，连我自己都不敢说现在算是成功了，因为后面还有更艰苦的临床考试，所以我很理解前辈们的苦衷，他们不是不愿意分享自己的经验，而是实在没有这个时间。我不想劝大家走这条路，因为个人情况不同，结果可能也不同，我不能向我的医生同事们保证这条路是正确的，我只是希望给大家一点线索，分享一下我的经历，也许能给大家一点帮助。

第三，生存问题。“我们要吃饭，养家糊口，一人打工，一人带孩子，哪儿有时间看书考试？” 在这里我要举一个例子。如果大家都看报纸的话，（注意不是中文报纸，而是主流英文报纸），也许记得去年6月份的一篇采访，一个非洲国家好像是索马里的医学毕业生，24岁来到多伦多，一边打工一边读书考试，开过出租车，送过比萨饼，领过救济金，10年内考过了TOEFL,TSE,MCC笔试，3次考进OIMGP的口试但是都没有入选，大家都说他的理论知识扎实，可惜每年安省只有24个名额，而他每次都是考过OIMGP的笔试，到口试的时候被淘汰的。去年年初安省扩大了名额，他终于如愿以偿，现在已经当上住院医生，2年后就有望独立行医了。请问大家谁有他这样的精神？生存的确重要，但我们难道要打一辈子工吗？

第四，语言问题。中国人的英语还是不错的，看看北美各大名校的奖学金得主就知道了，但是口语普遍偏差，不敢和别人交谈，装电话、办上网、买车、买房都喜欢找华人经纪，专业单词更是只会认，不会读，不敢和本地医生交流。其实这才是最简单的问题，学习语言贵在坚持，只要付出就会有回报。听听印度巴基斯坦医生的英语吧，相信可以增加大家的自信心，要知道加拿大有25%的医生是IMG(美国加拿大以外国家毕业的医学毕业生)，而其中一半是印巴人，他们的英语并不比我们好多少。

原因还有很多，但是我想最主要的是以上几个。我会在以后慢慢分析，分享一下我的经验，帮助大家克服这几个难关。

“对于移民来加的医生，加拿大并不说不承认你的学历，只是要有加国的行医执照。（在医院工作或个人开业都需要此执照）在安大略省取得此执照的过程如下：
1.通过加拿大医学理事会的评估考试
2.参加安省为国际医生开办的研究生课程。要上此课需：托福580分；TSE（英语口语考试）50分；入学考试第一部分，笔试6小时，临床医院选72人；72人参加入学考试第二部分选24人；这24人参加为期36周的内、外、妇、儿科的课程学习，每科学完后都有考试，若有未通过者可延长12周学习。
3.通过加拿大医学理事会资格考试。第一次考试；为期一年的住院医训练；第二次考试；通过了所有上述考试，医学理事会发给证书，安省医学协会发给行医执照。有了执照离找到工作还有很长的路要走。您想来试一下吗？
所以，对于我们已“走上正规”的人来说，有大量的书要读，有许多试要考，我们最缺的是时间。
......我妻子一九八六年毕业于北京医科大学医疗系。来加拿大先学英语，再转学计算机编程。一九九九年秋找到程序员工作。”

事过境迁，我不想评论王前辈的分析，能告诉大家的是，现在情况已经有所好转。我很忙，请不要给我发Email，因为我没有时间回信，如有问题请跟贴，我可能回贴很晚，还请大家原谅！下面先简单汇报一下我的经历：

1.下定决心考执照，搞清楚了必要的步骤，但是如何准备考试没有一点头绪；
2.顺利通过TOEFL；
3.顺利通过MCCEE；
4.顺利通过MCCQE Part1笔试；
5.顺利通过MCCQE Part2口试；
6.顺利通过TSE；
7.顺利通过OIMGC笔试；
8.顺利通过OIMGC口试；
9.顺利通过OIMGC面试；
10.得到住院医生Offer。

学习英语最好住在小镇上，我只有考试的时候去过几次多伦多，发现那里华人太多，好些时候不用说英语都能办事，这实在不能说有利于我们提高英文水平。

如果你能读出护士来，你的英语一定没问题，足够考医师资格考试，关键是很难拿到住院医生的实习机会。这就需要下一番苦功了，因为你要和来自世界各地的IMG竞争安省的每年200个名额，或者全国的每年80左右名额。

1.What material can I use for studying?
The following url (http://www.mcc.ca/pdf/reference_list.pdf) contains a list of reference books that you may find helpful when preparing for the examination. In addition, the MCC sells a book called the “Objectives for the Qualifying Examination” that will be of useful to you (http://www.mcc.ca/english/publicatio...bjectives.html). The MCC also provides a practice examination for the Evaluating Examination and the MCCQE Part I. This practice examination can be ordered on-line through the MCC web page. Alternatively, a demonstration of the MCCQE Part I can be taken on-line (https://www.mcc.ca/MCQ_Demo/start.htm) and contains questions similar to what you would receive on the Evaluating Examination.

2.How do I use the “Objectives for the Qualifying Examination” for studying?
The Medical Council of Canada suggests that using the “Objectives for the Qualifying Examination” (http://www.mcc.ca/english/publicatio...bjectives.html) as a study guide may be helpful for examinees trying to pass the Evaluating Examination. Note that the book is organized in terms of clinical presentations. Under each clinical presentation is a set of learning objectives that are exactly what you, as an examinee, have to know for any of the MCC examinations.

When studying for an examination, systematically go through each clinical presentation and carefully read the related objectives. If you feel that your knowledge or skills related to a given objective is weak, then you know that this objective needs further study and will require you to consult a medical reference/textbook. To aid you further in your studying, the objectives are grouped in terms of medical disciplines. If you feel that you need extra studying in a particular discipline, you will be able to identify the relevant clinical presentations within that discipline.

Finally, the MCC test committee members use the clinical presentations and learning objectives listed in this book when creating test questions. By reading and familiarizing yourself with this book, you will be able to deepen your knowledge of the information that is required for all of the MCC examinations.

3.Should I use the MCCQE Review Notes (Toronto Notes) for studying?
Many people also have access to a document called the MCCQE Review Notes (Toronto Notes), which contain study notes and a list of multiple-choice questions. Please be aware that this document is published by the graduating class of the University of Toronto and is a collection of information that these students feel is relevant. The information within has not been surveyed or ratified by the Medical Council of Canada and therefore this document is not a publication that is endorsed by the MCC.

4.Should I take a review course offered by a training facility?
The information offered within these review courses have not been surveyed or ratified by the Medical Council of Canada and therefore taking these courses is not endorsed by the MCC.

5.Should I use one of the existing ghost banks of multiple-choice questions for studying?
If you are unfamiliar with multiple choice question examinations, testing yourself with questions from a ghost bank may provide you with useful practice. Please be aware, however, that these are not official MCC questions and that there is no guarantee that the information within the question is correct. In addition, there is no guarantee that the questions in a ghost bank are reflective of the content from the MCCEE or that the ghost bank covers the breadth of content that occurs on the MCCEE.

6.What is the relation between the MCCEE and the MCC Qualifying Examination Part I?
The MCCEE and the MCCQE Part I are similar examinations in that both examinations test basic medical knowledge and both use multiple-choice questions. There are some important differences, however. Uniquely Canadian content is not tested on the MCCEE but is included on the MCCQE Part I. Questions dealing with Canadian legal issues in the practice of medicine are also not included on the MCCEE. In addition, the MCCQE Part I includes a one-half day section on clinical reasoning/decision making. There is no such section on the MCCEE. Also, the MCCEE is administered at seven international sites, thus MCCEE candidates do not need travel visas to enter Canada nor do they need to bear travel costs to and from Canada. The MCCQE Part I is only administered in Canada.

In addition, the MCCEE is not designed to purposely bar bona fide examinees from entering the MCC Qualifying Examination stream en route to Canadian/Provincial licensure, and as such, the passing requirement is much less stringent for the MCCEE than for the MCCQE Part I. In other words, the MCC does not want the Evaluating Examination to be an artificial barrier to International Medical Graduates who demonstrate medical knowledge beyond a minimal competency, but rather to be a screening examination to allow the EE candidates and the MCC to assess/evaluate a candidate’s readiness to enter the MCC Qualifying Examination and/or further medical education experience in Canada.

7.How many questions are there on the MCCEE?
There are a total of 324 questions on the examination that are presented in two booklets containing 162 questions each.

8.How much time do candidates have to complete the MCCEE?
The examination is a full-day examination. These test items are administered in two 3½-hour sessions (one in the morning and one in the afternoon).

9.Will studying for the United States Medical Licensing Examination (USMLE) Step examinations help with the Medical Council of Canada Evaluating Examination (MCCEE)?
There are some similarities in the examinations. The USMLE and the MCCEE examinations use multiple choice questions that are similarly structured. The USMLE and MCCEE present questions that have a patient problem in the stem followed by a question relating to that problem. The question is then followed by five options with one of the options being the correct answer.

There are also some important differences between the USMLE Step Examinations and the MCCEE. The USMLE Step 1 tests basic science knowledge that is not explicitly tested on the MCCEE. The Medical Council of Canada assumes that knowledge of basic science is needed to answer the questions in the MCCEE. The USMLE Step 2 and the MCCEE test clinical knowledge at the level of someone going into supervised practice. However, the material tested on the USMLE Step 2 may not be the same as the material tested on the MCCEE. The USMLE Step 2 tests information along two dimensions: Normal Conditions and Disease Categories, and Physician Tasks. The information tested on the MCCEE is based on the clinical presentations found in the Objectives of the Qualifying Examination published by the MCC (see www.mcc.ca). The USMLE Step 3 tests the clinical knowledge of someone who is ready for independent practice and is, therefore, testing different material than the MCCEE. There are also differences in scoring and reporting procedures between the USMLE Step examinations and the MCCEE.

Like any two examinations testing the same content area, it is reasonable to expect that there will be some overlap of content between the USMLE and the MCCEE. As indicated above, there are also differences. It is best to check the USMLE.com website for the “Content Outline” for their examinations and the Objectives of the Qualifying Examination of the MCC for the content to be covered by the MCC examinations, and study accordingly.

不过我发现他们都有一个共同之处,他们大多是以访问学者的身份出来的,在这里读了些基础的MASTER或是PHD后,不愿意在实验室呆,再下苦功读临床的.

他们的成功,在于一,先读了些书,几年在大学搞基础,过了语言关.这里报考医学院,都是面世的,没有好的语言技巧,过不了关的.二,在大学混了几年,有了一些人脉,可以和主管招生的人联系上.

有个问题想请教Neverangry和大家，我有中国的医学士学位（Bachelor of Medicine），但是没有临床培训过 (因为专业是公共卫生). 如果想考加拿大行医执照中的Qualifying examination part II, 是不是一定需要至少有一年的postgraduate clinical training?

只要有中国的医学士学位（Bachelor of Medicine），就可以考MCCQE part I，但是如果没有临床培训过是不能直接考MCCQE part II的。你的专业是公共卫生，是不是实习了１年才拿到的学位？如果想考加拿大行医执照中的Qualifying examination part II, 中国的医学士实习１年 is considered postgraduate clinical training. Good luck!
